Jefferson Healthcare Center is a licensed Medicare and Medicaid skilled nursing and rehabilitation center located in Jefferson, Ohio. We provide specialized programs and rehabilitation services onsite to help our residents regain their independence and return to their homes.

Jefferson Healthcare Center practices a philosophy of care known as “Patient Centered Care” which empowers our residents to take control of their lives and personalize their own plan of care the way they see fit. Our model of care at Jefferson Healthcare Center seeks to de-institutionalize the standard healthcare setting of get-ups, schedules and appointments, and fosters a virtual home environment that is also comforting.

- We implement the most advanced wound care procedures in the industry
- All rooms have new electric high/low beds, and cable TV (private phones are also provided for our short-term rehab units).
- The full in-house rehab unit provides: PT, OT, ST.
- We have recently updated our therapy gym, resident multipurpose room, front lobby, chapel, and our transitional care suites on Dogwood Drive
- We have been serving our community since 1984
- Admissions can be processed 24/7.
- RNs are on duty and onsite 24 hours/day.
